Methods to
obtain medical records are
Apply for medical records, get yourself a standard
form (sf-180) and fill up the necessary details and
request for the military records. Then send a letter
along with the form to the vital records information
department. Also provide them of proof of your
credentials to attain that information
National Archives
go to the National Archives and Records
Administration's National Personnel Records Center (NPRC)
or the new Archival Research Room in order to attain
military record information. Before doing that you
should schedule an appointment well in advance so that

State or County records
office also you may be able to obtain
copies of certain military records from your state,
county or municipality offices. For that contact your
state or county veteran’s agency for more information.

usually provides copies of service records as a free
service, you may find that you require more extensive
research assistance, and may want to hire an
